Schedule Your Biometric Screening

Take 15 minutes to invest in your health and complete your biometric screening. Carefully select your preferred date, time, and location when registering.

Eligibility Reminder:
Screenings are only available to employees enrolled in the District health plan. Spouses and individuals not covered under the plan are not eligible. If you have questions about eligibility, please contact HR.

What’s Included in Your Screening:

Your biometric screening, administered by Saint Alphonsus, includes:

• Blood Pressure
• Hemoglobin A1C & Fasting Glucose
• Height, Weight & Body Mass Index (BMI)
• Waist Measurement
• Comprehensive Metabolic Panel
• Lipid Panel

**If your school is closed due to unforeseen circumstances, your scheduled screening will also be cancelled. You will be notified by email if this occurs.**

Alternative Option with Your Personal Provider

Prefer to complete your screening with your personal healthcare provider? You may do so by submitting a completed 2026-2027 Provider Form directly to Saint Alphonsus Corporate Health & Well-being by March 31, 2027.

Disclaimer:

During your biometric health screening, the following measurements will be collected: height, weight, waist circumference, and blood pressure. In addition, a venous blood draw will be performed to assess a Comprehensive Metabolic Panel (CMP), Hemoglobin A1C, and Lipid Panel.

All data will be compiled into an aggregate report and shared with the Boise School District Joint Insurance Committee at the conclusion of the program year. Individual confidentiality will be strictly maintained in accordance with HIPAA regulations
